Tank owners can now register underground storage tank information online using the myDEQ Portal instead of completing and mailing paper forms. Online registration will not replace paper forms, but is an additional registration tool for tank owners.

UST Registration Online Forms (access these forms by first clicking on the myDEQ Portal link above)

Owner Registration Information Form: Use this form to initially notify DEQ of a new UST owner or to make changes to the information of an existing UST owner and contact. If you need to change the current owner of a tank to a different owner you must use the Change of Ownership Form.

Update Facility Registration Form: This form is used to update existing facility and tank compliance information for individual facilities. This form should not be used to register a new facility or to change the ownership of an existing facility or facilities. 

Change of Ownership Form: Use this form to change the legal ownership of tanks(s) currently in use or temporarily out of use at a facility.

New Facility Registration Form: Use this form to notify DEQ of a new UST facility that has not been previously registered. 

Change in Multiple Facilities Form: Use this form to update UST system information that applies to multiple facilities - for example, if you are notifying DEQ of a change in release detection method at more than one of your facilities.

UST owners must file a registration form with DEQ within 30 days of any of the following:

• Installing a new UST system
• Purchasing a UST facility
• Changing the status of a UST system (for example, putting a tank or tanks into temporary closure)
• Permanently closing a UST or its connected piping
• Making a change to the tanks or piping (for example, installing a liner in the tank or adding a corrosion protection system)
• Changing the release detection method for a tank or its connected piping;
• Changing the substance stored in the tank

 Only a UST owner can submit registration forms to DEQ.

Form 7530-3: Use this form to register new tanks and any changes in the tank system or its status. You can also use this form to register the purchase of a tank facility or individual tanks at a facility. Click to download Form 7530-3 instructions.

Form 7530-3A: Use this form if you have just sold your UST facility and want to alert DEQ to the sale. This form is not complete and will not be reviewed unless it is accompanied by proof of sale documents. Click to download Form 7530-3A instructions. 

Form 7530-3B: Use this form if you own multiple UST facilities and want to change something simple that affects more than one of them. For example, you change your owner name or address. You cannot use this form to notify DEQ of an ownership change. Click to download Form 7530-3B instructions.

Form 7530-3C: Use this simple, one-page form if you are buying ALL of the USTs at a facility. If you are buying some but not all of the USTs, use Form 7530-3.
